Support to thrive at home
Individual Community Living Support (ICLS) offers essential assistance to individuals aged 65 and older enrolled in the Alternative Care (AC) program or the Elderly Waiver (EW). To qualify, individuals must receive support in at least two of the six ICLS service components:
- Active Cognitive Support:
Assists individuals with cognitive challenges, concerns related to daily living, and provides reassurance. Support includes redirecting cognitive or behavioral concerns, providing check-ins to identify and resolve issues, and assisting with orientation. - Adaptive Support Services:
Encourages self-sufficiency by offering verbal, visual, or touch guidance for tasks, helping with assistive technology, and developing cues or reminder tools (e.g., calendars, lists). The goal is to maintain independence through innovative strategies and continued self-sufficiency. - Activities of Daily Living (ADLs) Support:
Supports individuals in completing ADLs such as dressing, grooming, eating, toileting, mobility, and bathing. ICLS can provide reminders, cuing systems, and intermittent physical assistance, though it does not replace services like Personal Care Assistance (PCA) for continuous support. - Household Management Assistance:
Helps with cleaning, meal planning and preparation, shopping, and budgeting. Support also includes communication tasks such as sorting mail, making phone calls, and scheduling appointments, along with providing transportation when needed for household management. - Health, Safety, and Wellness:
Supports health and wellness by identifying changes in health, coordinating care, assisting with exercise and medication, and managing health equipment. ICLS providers follow written instructions from licensed health professionals to monitor health and report changes. - Community Living Engagement:
Encourages individuals to engage in the community by facilitating access to activities, services, and resources. This includes developing and maintaining an informal support system and providing transportation when community resources or informal support are unavailable.
Each service component is guided by the ICLS Planning Form (DHS-3751), completed by case managers/coordinators, to ensure that services are tailored to the individual’s needs.
